I still like the old National application notes.  AN20 is a classic and 
covers the most basic opamp applications in just a dozen pages.

AN31 is a collection of circuits, with no text.  The appropriate text can 
usually be located in one of the other ap notes  (same for many circuits 
given in data sheets).

For texts I like Walter Jung's two books "IC Op-Amp Cookbook" and "Audio IC 
Op-Amp Applications".
