[sdiy] Paia Ribbon Controller

Hi all,

I'm currently building the Paia Ribbon Controller as seen here:

I'm not sure what IC1 is - will a 4066 or 4016 do? I've tried these, but 
they don't seem to work. Looking at the datasheets, they seem to want a 
control voltage of Vdd but the gate only kicks out 9.13v. Also, I'm 
running it off +/-15v, not 12 as in the schematic. Could this be the 
problem? It works fine without the switches, but I want to use them!
Also, what are the points labelled ds4 and 5 on the Hi and Lo hold?
