I did a webpage on my favorite method:

I've also used laser/ink jet printable labels with tolerable results.

The hardest part with this method is building up enough clearcoat to smooth
the label edges. Look at the glare along the bottom edge and you can see
the gate/slide label edges. Its definatly low-cost tho.
I still use the water decals for stompboxes and stuff in weird shaped
standalone cases, but I prefer the look and ease of the ID-MARK stuff.
