It really helps a lot if you can have a standard aperture and drill  
file.

Go from small to large, with specials at the end.

You can flash the pad file to make a solder mask, and save having all  
those +10 mil apertures.

For special pads, change only a few apertures at the end of the file.

Watch out for rectangular pads, they use a funny spec.
