I have two the Paia midi/cvs.  They work well.  Its possible, with some
hunting, to find all the software you need to recompile the code so you can
make changes easily tho it does require a prom burner.  For example I've done 
an ugly hack to have it start the scale at F unstead of C so I can directly 
drive my minimoog.  as far as assembly, the wires to all the jacks drove me
crazy esp since I have them both in the rackmount case.  Beyond that it was
a quick build ( actually in my case it was a rebuild but thats a long
story... )

Thats a link to some early code for Trevor Pages' Midi/CV using the 16F84.
That code plus the EFM schematic should get you a simple midi/cv.  One great
thing about that midi/cv is its dirt-cheap to make.  Its not insanely
accurate as stated on the page, but its really cheap to make.
