Hi,

Additional to the ones already names by others:

Curetronic in Germany:
http://curetronic.de/
Kits and fully assembled, in 5 HE Moog style or 6 HE more or less  
Formant style.

A superb VCO is available as PCB from Anyware Instruments:
http://www.anyware-instruments.de/
But the documentation for that is in german only and the PCB is only
produced on request.
You probably know Anyware Instruments from the Original Simplesizer  
Project
(Elby Designs Hobbit is a copy of that).
Anyware also has made the HelLFO:
http://www.sequencer.de/forumsynth/lfo.html
which is a somewhat custom design for Moogulator, but I think the PCB  
is also
available on request.
