When I did my quad midi cv some years ago, I used a 
Burr-Brown PCM56P.
Now this was a 16 bit serial D to A, intended for the first
generation of CDs, and as such, it wasn't guaranteed for DC
offset. But, I never had any trouble..
I was only wanting 12 bits anyway (12 is the min for Hz/V).
I only mention this, because if you want CHEAP Dto A,
then consumer audio will be the place to look!
