I took some data from eproms, and wrote a little dos command line that 
'stretched' them to be 256 bytes.
Other than that, i use grants wave256 tools,

As for what size, well, as you say, there are pros and cons.
256 samples per wave is a fairly good sample rate when you play low notes 
(less harmonics),
for example at say 100 hz, 256 samples is 25.6Khz sample rate, with 64 
samples, its 6.4Khz..

The advantage of using 256byte waveforms is that you can always lower them, 
by masking off one or two LSBs in the address (the Monowave 'deres' masks 
off the two lsbs, and gives you 64samples per table).
