premade MBHP_OPL3 PCBs are currently only available from Mike for 6 EUR:

Tim is working on an improved version with laserprint and solder mask (nice
when stuffing the SMDs) for the same price - it will be available soon at:

He sells also PICs with preprogrammed bootloader, you can upload MIOS 
and the MIDIbox FM application via MIDI
