I would google for "cmos theremin" for an idea
of a circuit.  It would be crude but it will work
at near ranges.

PAiA has the Theremax theremin kit which would work
(maybe) better.

If you only need to sense a few inches of distance,
you
might try using a photonic approach like the Roland D
beam.  Marjan Urekar has the schematic on his site...
it is a lot simpler than a theremin
