I know I have seen (and probably have some of) a voltage follower in an 8-pin 
DIP,  but I can't recall the part number right now and my initial searches 
don't seem to come up with anything.

Consumer-grade stuff seems to use a fair amount of SIP packaging,  but again I 
haven't run across a number on those,  either,  though I did find a dual 
comparator in a SIP.

When I get more info I'll post about it.
