Favourite: Easy-PC (www.numberone.com)

Turkey: Vutrax ARGHH ARGHH ARGHH

Fabrication: you might consider Eurocircuits (www.thepcbshop.com) - I've 
found them consistently cheaper than PCB-Pool and PCB-Train and the quality 
is excellent.
