I just went through the process of figuring that out for a bunch of MOTM
kits.

Your list is excellent, although to reproduce the usual "analog
monosynth" normalization, you'll want a second ADSR so you can have
separate envelopes for amplitude and filter cutoff frequency or
something else. You'll almost always have one ADSR on amplitude, a
second one is nice.

Also, don't forget to have a few mults crammed in there somewhere.

Have you decided what to use as a controller?

If you can, find someone who has a modular, and try patching it for an
evening or two.  You'll get a really good perspective this way.

You may want to listen to a bunch of different kinds of filters too,
before deciding.  A lot of the character of a sound comes from the
filter.

Upon speaking up on the MOTM list with a question like this, I was
carefully instructed to make a web site with pictures of what I was
building, and to give my synth a sexy name.  I hereby advise you
similarly.

As for the rest of your questions, I'll let one of the more legitimate
geniuses here reply. :)
