it should be possible to order small PCBs in any PCB fab. Even as small as 1206 resistor. They just cut the PCB on both sides leaving thin part of the epoxy, so entire big PCB is in one piece, but you can rip the pieces out easily by bending.
If you don't find any fab near you, I could recommend a good cheap fab in Poland
