[sdiy] converting dac output from 5v to bipolar 12v for audio

hey everyone.. 

 

just starting a new project..  I plan to run audio in and out of a pic 18F452 and manipulate it in various ways..   i've got a dac which is putting out 0-5 right now..  i'm going to change the vref to 10v when my new v-refs come in..  anyway, i'm looking to convert some signals..  input will be standard audio of various sorts which will NEED to get converted to 5v for use with a 10bit adc..   then, i need to convert back, from 5v to +/-10v.. although, i will be changing the vref to 10, so i need it to be somewhat adjustable..    anybody got a schem for an opamp circuit to amplify and offset said signal accordingly?
