I've implemented a MIDI operating system which already does
all the MIDI and IO processing to simplify the realization of the 
application itself (it is isolated from the OS) 

In the MIOS download section you will find an example how to
drive CV outputs with this OS, I've also created an advanced
application for driving 8 CV outputs with independent note stacks,
poly mode etc.:

And the MIDIbox64 application supports up to 64 CV outputs :)

All applications are open source projects... feel free to customize
the applications for your own needs.
