I think they might have made more than one. But, what you are describing
sounds exactly like the Yamaha MCS2 (Midi Control Station 2). It is a wedge
shaped device and it is the right size to next on top of a TX-7.
The MSC2 has pitch bend, mod wheel, two assignable sliders, 3 assignable
push buttons, 8 buttons for sending patch change (yes, you send 1 through 8
bank, and 1 through 8 patch), 2 assignable foot switches, and 2 assignable
expression pedals.
It has 2 MIDI INs and one MIDI out. The 2 MIDI INs are merged with each
other and the control data in the MCS2. These are great little boxes when
using older MIDI keyboard that do not support all these features as
controllers. I used one on stage for many years with my Korg DSS-1 which
had no pedal function for volume. I found the MIDI control of volume always
the best as my set up often layered sounds from different modules. I used
the patch change feature to key up the right MIDI patch in my MX-8 MIDI
patcher, which then set every module on the correct patch for that
particular song.
The only drawback to this device at all is that all of the assignable
controllers were selected to the same channel. Channel assignment was
global.
See, you do have someone to ask. I still own two of them. One for gigging
if I ever go back to that, and one for a spare. Yep, it was valued by me
enough to have a dedicated spare.
