I think "DSP Filters" by Lane/Datta/Karley/Norwood, ISBN 0-7906-1204-6 
fits the bill. It is rather basic, but at least gets far enough to show 
you working code for DSP filters. Working, not necessarily efficient or 
good. The price is a bit steep for what it offers, so try to get at 
discount, buy used or even just look into it in a library.
