The guy who designed a 10-bit DAC board with 5 bits per word
also designed the electronics for a dot matrix printer using
an unregulated 48V power supply and never bothered to check
if the peak voltage exceeded the max Vce of the optoisolators
he used to couple the digital port to the drive electronics.
One night he burned up our entire stock of power transistors
and microfuses before I arrived and debugged the problem. He
was demoted shortly afterwards.
