Mine is a JW Miller 5250 100uH (not mH) 2A choke...
 
I'm not recommending that one, I just used it.

The etherwave uses four chokes in series... they are
RF Shielded types (with ferrite overmolded). I'll
check the values for you, but have no part numbers
