Circuit Cellar magazine happens to have an article on 
building a Class D audio amp based around some chip who escapes me
right now.  The parts count was extremely low and it went into a least
a bit of detail on theory and realworld application.

Hrm. Or it was Nuts&Volts.  I will look when I get home.
I cant seem to find the article listed on circuitcellar.com except for the
All Digital Audio Amp article which is in a very new issue of the
magazine.
