simple amplifier and/or comparator,
e.g. gain of 1.0 or 300mV threshold with some 10mV hysteresis,
frequency 1mHz-20Hz. +-15V supply.

I've used RC4136's recently, because I needed noninverting,
latch-free audio amps in quad package. 
I'm not sure if Vos is < 5mV, though.
