It's often due to the guy with his fingers on the mixer controls.
The number of beer bottles at the mixer desk is a good indication.

To be fair, a lot goes wrong at a concert event, and it's really hard
to debug this stuff on the fly.  Fripp calls it "the law of maximum
distress".  Equipment connected wrong, inappropriate levels, clipping,
ground loops, ac noise, bad cables, dirty connectors, broken
equipment, dust, beer spills, etc.

Heck, one show I played, the PA System was stolen a couple nights
before, and they were working on newly borrowed equipment.  'So much
for all those concerns about phase alignment.  :-)
