[sdiy] Midi2CV Update

Yo,

First, thanks to all who sent suggestions on panel
fillers. I am happy to say the panel got filled. I
settled for CV, velocity, pitch bend and mod outputs,
gate and trigger outs, separate mod and PB outs with
amplitude pots, and 4 each cv and gate outputs that
decode from controllers.

After a long battle with this thing, I finished
assembling the final module. It looks kinda scary,
with its 4 baords and 3" wide module. Alas, it didn't
work. I found some small problems, and ... it doesn't
work. 

So I guess its time to reach into the ol'
troubleshooting kitbag:

"Let's see here: meter, scope, dead chicken, Holy
water, blow torch..."    ;-)
