No.

Yes, but pan was never formally standardized AFAIK, so it could be any 
controller (the usual suspect is of course #10).

I sense that you might be after polyphonic panning (otherwise the 
inclusion with the note on would make no sense). This only works if the 
synth in question samples the controller data on note on and ignores 
further changes to the controller. In that case you need to send the 
controller just before the note of course.
