I have used old PC keyboard extensions as well, but I've never known if the
shield pin is actually the cable's shield nor do I know if that pin is
connected at one end or both. That said, I've had no problems either with
the MIDI stream or any noise being emitted by the cable.
I have also made my own extension cables as well as standard type MIDI
cables. They aren't that hard to make, the spec. is well known.
