I think that this is as simple as it gets:

Why such the rage for a simple theremin?  If you're looking for an easy 
synth with a non-standard controller, maybe the synth-stick is the way 
to go:

THey're a lot of fun, and you really don't get much simpler than a cmos, 
two caps and a pot.
