Dumb answer: with solder and a soldering iron.

Longer answer: I have soldered far too many PCBs with SMT parts 
including 0604s and 100pin SQFPs (0.5mm pitch).  The tricks are to 
use really fine solder and a very fine soldering iron, and often a 
lot of flux.  0805s and bigger are actually easier and quicker to 
deal with than through-hole components.  You don't have to bend or 
trim the leads, and you never have to learn the resistor color code.

Some people even do the solder paste thing at home, but that's a bit 
more of a pain because the paste only stays good for so long and you 
need decent control over temperature.
