Use the same gauge wire as in your power cord.


It then says to put the AC at jumpers 1 and 4,

Either way is fine.

Between the "hot" wire coming in on your power cord and the 
transformer (point 1 or 4). Connect the neutral wire to the 
other jumper.

Jumper +S to +OUT and -S to -Out. COM connects to your 
circuit grounds, +OUT and -OUT connect to + and - 15Volts 
respectively.

Another question is

No (see above for ground connection), the third prong of 
the outlet wire gets bolted securely to the metal chassis 
of the power supply.

Its a good quality supply and should work well for you. You 
may want to consider fusing the outputs as well.
