Hi Paul,
I found some nice looking ones for my MIDI2CV, made by DELTRON.
Black, 5pin, 180deg part no. is 651-0500.
Personally I got the silver (650-0500) since I think they look really good mounted on a black panel., and match those switchcraft jack sockets. Also, since the inner part of the socket is grey, not black, the silver is less contrasting.

I'm not sure if these are the same as Mouser #164-2522 (http://www.mouser.com/catalog/612/420.pdf) though - in UK they are only £1.35 for 100+
