on corrected_amp_schematic.gif change pin 5 and 6 at 4558,
5 should go to + and 6 to -, mind that + and - signs
are on the correct places AFAIK.
On the pcb you should move 47k pot one position to the right
because you need it's wiper to 1458 pin 2, not as you drawn.
There's one large pad empty all to the right, that's where
you should move it.
Line input is OK but rather unusual design. I'd add another
10k to ground from 4558's - pin.
I don't have more time right now but I'll take a more
thourough look later,
also, I don't get biegegreen and yellow dots
