In all likelyhood, you are looking at a "spider" a two stage
divider. Similar devices can be seen at
http://www.SystemsInfo.com/topdividers.html#Fig11
You *may* be able to get one from an organ repair supplier,
or you may have to build yourself a replacement like I did.
I used a 4013 CMOS divider IC feeding into transistor
buffers.
