[sdiy] Min. Bd. 1 Osc. g1-113G MOOG

Hey Gang,
This is the type of minimoog osc board that I have.  Could someone tell me if it is first, second or third issue/revision?  Also I need to connect it to some jacks switches and pots...also power supply fo a home in my modular.  I am considering getting some connectors from Fred McNiff..... question is.... where do these connections go...... I need a scematic or labeled layout of the mini osc..... any help would be great.
