Could anyone tell me what this is?  I do know that its a 22 pin ic that is smack dab in the middle of my noisy MXR Stereo Chorus (the big yellow box with built in AC cord.  Sources and availability amongst other info would be great.

does anyone suppose a faulty three pin 7812 op amp could be my problem?
I'm going to go see what the symptoms are again.
