I cured this with a 0.01uF ceramic, you have to try a few different values

I think there is an overlap on the pad next to Pin 1 of U6 4069 connecting it to pin 4-
maybe this is causing some of your other problems??
I also ended up having to change R46 & R41 to 10K to enable correct functionality,
I heard of no one else having this problem so I dont know what went on there??
I hope you are using blue LEDs?
