Those narrow phone plugs were actually used on every Minimoog.  There are two 
of them on the left hand control section, right next to the pitch bend and 
mod wheels.  They were used for connecting footswitches to the glide and 
envelope decay (release) functions.  Moog sold special footswitches for the 
Minimoog that had those plugs on them.  I guess they were trying to make sure 
that you wouldn't short out the circuitry by accidentally plugging in a 
standard reverb/tremolo on/off switch from a guitar amp.  I'm pretty sure 
that they were 3-conductor (TRS) plugs and jacks.  I'm sure that Switchcraft 
still makes them, although you probably won't find them at your local 
electronics store.
