  *yes*.  You have to drop whatever you're doing to pass along or process
the realtime byte.  A realtime can happen inside a message, e.g,
$90,$4C,$F8,$40.  The state machine for grabbing realtimes combined with
the state machine for keeping track of running status is not too tricky.
I've got (*old*) Z8 code I wrote 15 years ago that illustrates it; I'll
dig it up off the cp/m machine, if I can get it to boot.
