Funny enough, quite a lot of pot manufacturers have one or the other
concentric pot in their catalogue. They are probably selling them
to car radio manufacturers, or at least they want to show in their
catalogue that they can build something like that, in case a car
radio manufacturer is interested. (How many car radios still
have these nowadays, btw. ?) Only that you will most likely not
be able to buy small quantities such as 1 or 100.
As for the knobs, that's much less of a problem.
You can buy Rean collet knobs in different sizes, and the next
smaller size knob will exactly fit on top of the next larger
size knob (where the cap would normally go).
Ever wondered why the collet has a hole ? That's where a 4mm
pot shaft fits in.
Maplin had them last time I checked. Used them for the JH-720
(though not concentric).
Elma knobs can do the same thing of course. (More expensive,
better feel of surface, IMO)
