I have used the ADC0801,2,3,4,5 A/D from National for years and find that
they work well for this kind of thing. The coolest thing though imho would
be to use a PIC Microcontorller that has a few analog inputs already. You do
have to program it though but that way you could have the module be MIDI
controlled and maybe implement a whole bunch of other MIDI controllable
features and lose the CV altogether.

As I recall you can put your finger across the crystal and get the pitch to
change so I guess it is possible.
