Hello Mark,
That chip is usually referred to as a CD4067. Mouser Electronics has that
part. It is Mouser part # 511-4067. Their price is $2.72. (shipping will
probably be about $3.00). They have no minimum order $ amount, bless their
hearts.
Call them at 800-346-6873 (mon-fri 7AM to 8PM, CST), or find them on the web
at http://www.mouser.com
The part is also carried by Jameco Electronics, who have nationwide
distribution of their IC's in hundreds of electronics stores. Call your
local electronics stores (not Radio Shack...) and ask if they carry the
"Jim-Pak" IC series. If they do, they should have the CD4067, or at least be
able to get it for you from Jameco.

What can cause it to fail? Most likely, it was probably just a bad part,
destined to fail sometime - possibly just borderline out of spec. Or maybe
it took a big static electricity hit from someone touching the front panel
after shuffling their feet on a shag carpet for an hour (CMOS IC's such as
the 4067 are more sensitive to static electricity than non-CMOS parts,
although the greatest danger is when the chip is being handled out of the
circuit board). All things considered, I wouldn't worry about it. Put the
new chip in there, and it will probably work fine for the life of the synth.
