Multi axis foot pedals?

Another option would be to use a theremin-style controller, PAiA's Theremax
has pitch, gate, and velocity CV outs, and I believe that the more expensive
of Dr. Moog's theremins has midi and cv outs. There is an interesting
picture on PAiA's page of a guy who has three theremins hanging off of the
front of a motercycle that he uses to control his guitar effects with.
Also, Doepfer (sp?) has a theremin and a light->CV controler in modular
synth modules.
For a pedal controller, it might be an idea to have two narrow pedals beside
each other, so they can both be controlled by one foot. This may not give
quite as much independance, but would likely be more stable.
