Paul,

This is a General Electric 4N25 opto-isolator manufactured 
in the 28th week of 1989, I believe. NPN transistor output, 
good to about 300KHz. Pin 1 is LED anode, pin 2 is cathode. 
Phototransistor emitter is pin 4, collector is pin 5, and 
base is pin 6.
