Now I know. It's easy with separate gate and trigger inputs at the
envelope generators. The gate applied to the EGs is just the
gate/trigger from the voice assignment logic ORed with sustain. Much
like the hammer and damper of a piano string.

Something similar should be possible with sostenuto: it's like sustain,
but only the keys pressed before the pedal is pressed are sustained.
