Identify Slide Switch?

Hello done-it-yourselves,

I am trying to find a supplier for a certain type of slide switch.  It is the 
type of slide switch that is used in old Oberheim stuff, circa mid-70's to 
early 80's.  It was used in the Oberheim SEM and related products, such as 
the four-voice and two-voice.  It was also used in the Sequential Pro-One.  
Plase take a look at the attached GIF, and let me know if you know where I 
can find this type of switch.  The switch terminals are about .25" apart.  
There is a logo on the side of the switch, but I don't know the company it 
designates.  If you know the name of the company, that might help me during 
my search.  This switch seems so generic, that I can't understand why I 
haven't been able to find one yet.
