What is it?
It's a "voltage controlled wave select switch" with a little rewiring it's
a "bcd flash converter". 

How does it work?
A pair or LM3915s( or 3914s) (dot/bar display driver) are connected 
together to allow for 15 output states. This is decoded by 4 74LS30s 
( 8 in NAND gate) and applied to a 4016 (analog switch) to select a 
wave or combination of waves as the voltage goes up. Note the 4016
is connected to +9V/-5V. This can be done with zeners and 330 ohm
resistors if you like.

Why?
It allows simplification of panel designs. You can select any wave
combination with one pot and 4 leds.

I tried the John S. flash converter but I wanted a little more than it
could do. Switching saw, pulse,tri and sine in order for instance.
This circuit is the result.

BTW this is a 16th state (or 0th state if you wish) all off 0000 
when cv = 0
