Moog Source Midi Modification

A month ago I bought a moog source from this guy and when i got it, it had 
the cv/midi  modification on it. It has like 5 jacks on one side, and the 3 
standard jacks on the other. I have no idea what the 5 jacks on the left side 
of it are for, they are not labeled or anything. I heard that all i need to 
get is a cv to midi converter, but i have no idea which jack is which. can 
anyone help? thanks
