Master module

Hello all,

I intend to make a "master" module for my modular, containing a
simple stereo mix and stereo outputs and a headphone amp.

Have anyone else made this kind of module? (probably yes.) 
If so, how did you build the headphone amplifier? I'd like
mine to drive at leas 32 ohm phones, at a fair volume.

And how about balanced outputs? Ought I use transformer-balanced
XLRs or some sort of op-amp circuit? What is the best solution here?
