I believe that you can order a photocopy version of it from Hal
Chamberlin directly, and that there is a link on the PAiA page. 
Otherwise, you can track it down at a used bookstore - I bought my copy
at Powell's Technical Books in Portland, Oregon, and they have copies in
occasionally.  Be sure to look for the 2nd edition (the 1st edition came
out in 1980, while the 2nd edition came out in 1985 and has far more
info).
