Found It! National app notes "The Monolithic Operational Amplifier: A
Tutorial Study" Dec 1974. James Solomon calls it a "linear gain cell". I
wish I could understand the surrounding text but it's way over my head has
to to with the reduction of gm and fabrication techniques. Anyway I feel
better now. I looked through some more app notes and it is everywhere, now
that I know what I'm looking at.
