I used to be a member of the MIND-L list that had several people
interested in building their own devices like/including this one. I don't
know if anyone added MIDI control or anything but it should be doable as
it is/was an open design at the time. I have built a couple HAL's of
similar design (Micromint/Circuit Cellar) but never tried using them for
midi controllers. The thing is that the results aren't very impressive to
anyone else but the person it is all hooked up to. Its rather like
playing a theremin with your mind. I think there is is much that can be
done in this area to use your mind to control 3D audio environments, I
just never will get the time to work on it. But the goal is to come up
with a machine that enhances the experience of using it rather than
something that will help generate mind-controlled music for others. You
could network such machines though and probably interact with others in
the 3D environment.
I've gone to a few digital media "events" that had avante garde artists
from all over and a couple of them did use EEG control for their work.
But I sure didn't care much for the bleeps and such that were the result
of it.
You might want to look for an GSR (galvanic skin response) based game that
was out last year or so. It had a thing you attach to your finger and
then control a player on the computer - like a skier down a slope etc.
They said later it was all a bunch of crap - creative programming - but it
fooled me anyway - I thought it worked. When I wanted to go left it did.
When I wanted to go right it did..... Use the same concept and attach it
to the breath controller input of say a DX7 and you have a midi output
that you can control anything else with. (you can use the foot volume
control input too on this an other synths).

There is a internet project to design and build an inexpensive
EEG. See http://www.brainmaster.com (this seems to be down at the
moment, try http://www.nacs.net/~tomc/ ). It's supposed be an open
design, though I've been unable to locate schematics.
Don Lancaster mentions an application in his _Active Filter Cookbook_
using bandpass filters for EEG.
See http://www.nacs.net/~tomc/brain/vision/freqs for a list of
frequencies and associated states.
